How does the self test work?

The Cottle manoeuvre can be used to identify functional tightness. Place one or two fingertips on either side of your nose on your cheeks. Gently push and pull them outwards. This will temporarily stabilise the nasal valve. 

What does the self-test prove?

If this makes it easier for you to breathe in through your nose, the nasal obstruction is probably in the nasal valve at the front of your nose. 

It proves that your breathing can be improved by stabilising the nostrils, as the nasal valves can no longer collapse.

Advantages of optimized nasal breathing:

Breathing through the nose is important to humidify, purify and warm the air we breathe. By improving nasal breathing, poor mouth breathing can be reduced.

Mouth breathing can be responsible for various sequelae such as olfactory dysfunction, chronic sinusitis, chronic pharyngitis, laryngitis and bronchitis. Inadequate nasopharyngeal ventilation can result in persistent tube ventilation disorders and primary snoring (rhonchopathy without breathing pauses).
 

Why optimized nasal breathing is so important:

  • The respiratory air is dispensed through the nasal cavities. The respiratory rate (breaths per minute) is slowed down and this improves the total lung volume.
  • The fine cilia in the nose purify the air by filtering pollutants, dust and germs.
  • The nose is our air conditioning, the inhaled air is preheated and moistened in the nose, especially important in cold, dry winter air.
  • Nasal breathing with a closed mouth increases the flow resistance of the breathing air. This stimulates the important diaphragmatic activity and promotes diaphragmatic breathing (abdominal breathing).

Nasal Dilators Study:

Nasal dilators were developed over a century ago and were gradually introduced in the 1990s. They were very popular in 1996 at the Olympic Games in Atlanta (GA, USA) (1-3). A nasal dilator is considered efficient if it can alleviate sleep disturbances and snoring. The mechanism of action is based on a reduction of the nose resistance.

Nasal dilators may be useful during exercise, as reduced nasal resistance may result in a reduction in nasal breathing effort, increased nasal ventilation, and a delay in the onset of oral breathing during exercise (4, 5).

Many studies have been performed on athletes, mainly adults, using nasal dilators, as recently examined by Dinardi and colleagues (3). However, the results of these studies are contradictory and no final agreement has yet been reached.

NOSON® is a new internal nasal dilator that significantly reduces snoring (6). Therefore, the current study examined the potential benefit of an internal nasal dilator in a group of athletes.

The nasal dilators are a non-pharmacological treatment for constrictions of the nose and can be used by athletes, as it has been shown that nasal dilators can improve breathing and thus the athelete's resilience (7,8). In this regard, there is a number of cases of usage of nasal dilators by athletes, but the results are contradictory. Therefore, we investigated the potential ability of a new internal nasal dilator (NOSON®) to improve the athletic performance of a group of athletes.

The results showed that NOSON® significantly reduced the feeling of fatigue and was optimally accepted. Cardiorespiratory parameters were unaffected by both devices.

The current results are consistent with some recent reports. Dinardi and colleagues compared an external nasal dilator with a placebo nasal strip in 48 healthy adolescent athletes who completed a 1000 m race (7). The results showed that the external nasal dilator was significantly superior to the placebo one and it improved the maximal oxygen uptake, the nasal patency, and the respiratory effort. Another study by the same team investigated an internal nasal dilator compared to a placebo dilator (9). The study found that the internal nasal dilator significantly improved the nasal patency in adolescent athletes. In another study, adolescent athletes with and without allergic rhinitis were studied with an external nasal dilator and a placebo (10). The results showed that the external dilator significantly reduced nasal resistance, improved the maximal oxygen uptake as well as the perceived stress after a maximal cardiac breath test. The nasal device was effective in both healthy adolescents as well as the ones with allergic rhinitis. In a recent study, 13 healthy triathletes without any nasal symptoms were tested on and therewere 3 different conditions that were randomly tested: no nasal dilator, two different external dilators (11). The authors demonstrated that the two nasal dilators had similar effects, improving both the perception of the nasal patency, the nasal breathing time and the VO2max of the nose.

Therefore, the current study agrees with these reports and confirms the reliability of an improvement in the patency of the nose and thus an improvement of nasal breathing.

On the other hand, this study has some limitations, including open design, the limited number of enrolled subjects, the lack of follow-up, and the lack of validated objective parameters. Therefore, current experience should be confirmed by further studies designed according to a more robust methodology.

In summary, the present study shows that NOSON® is an internal nasal dilator that can significantly reduce the perception of fatigue and is optimally accepted.
